Sign-In Form

The Sign-In form is where we collect important info about our guests. It has all the standard name, address, phone contact, etc. It also asks for your email address. This is very important as this is now our main form of communicating with our guests. Before I go any further, let me assure you of a few things. Specifically our privacy policy. It's actually quite simple. We will never, for any reason whatsoever, share any of your personal information with anyone. And if at any time, for any reason, you no longer wish for us to communicate with you via email, you may remove yourself from our contact list with one simple click of your mouse. No questions asked.

We also like to know whether or not you've been a guest of ours in the past and whether or not you are travelling as part of an organized group.

All adults (18 and over) travelling on a Wild Dolphin Adventure package, whether you are snorkeling in the water, or just planning to ride along on the boat as a passenger, must fill out their own individual Sign-In form. You may fill the entire form out on your computer using the boxes/fields provided. Once you are done, you will need to print out completed form(s) for the next step. In all likelihood, unless you have Adobe Acrobat Professional installed on your computer, you will not be able to save a copy of the completed form to your hard drive.

Submitting the form

Once the form has been filled in and printed, you have a couple of options as to how you may submit them to Wild Dolphin Adventures.

  1. If you choose to submit them electronically, and, you have access to a scanner, simply scan the Sign-In form and email it as an attachment to forms@wilddolphins.com .
  2. The other option is to simply bring the completed form with you and give it to the staff upon your arrival.

Either way, you still save considerable time over the traditional method of filling out forms upon your arrival.
